ISU OPT 6 Month Report

This form is used for students on an OPT STEM Extension. International Students on STEM Extension are required to report to ISU their employment status every 6 months regardless of whether or not anything has changed. Please enter the following information every 6 months starting 6 months from the day your OPT was extended.
