Thank you for your interest in completing a registered Community Health Worker (CHW) Apprenticeship in Idaho. Please provide the following information to determine apprenticeship eligibility and placement. The competency-based apprenticeship can be completed on a part time (20+ hours/week) or full time (40 hours/week) basis, and is a total of 2000 hours. You may qualify for up to $7500 in living stipend paid to you over your apprenticeship time in addition to your regular employer pay.

Once this form is received and we verify your eligibility for the apprenticeship, you will receive an approval. Please allow 1-2 weeks for processing, and contact chw@isu.edu with any questions.
